Receiving health care in a familiar environment at home can be more comfortable, calming and convenient than being treated at an office, rehab center or other clinical setting.

At Home with Shepherd provides individuals with a full range of medical, rehabilitation and personal care services to help them recover, regain independence and return to their regular activity levels. We offer comprehensive and customized home health services to patients living in Trumbull, Mahoning and Columbiana counties.

About Home Health Care

Home health care is often ordered by a doctor following a surgery, hospital stay or to treat a recent illness or injury. Home health care services can be provided in the home rather than a hospital or skilled nursing facility.

At Home with Shepherd provides a full team of health care professionals including nurses, aides, and therapists to care for individuals in their home.

These trained health care providers collaborate with doctors to administer home health services such as:

  • Physical, occupational and/or speech therapy
  • Wound Care
  • Injections or IV Therapy
  • Nutrition Therapy
  • Monitoring Patient Health
  • Patient and Caregiver Education

Home Health Care Plan

We believe in a multi-disciplinary treatment approach. Our At Home with Shepherd health care providers will work with doctors, therapists and other professionals to develop and execute an appropriate treatment plan for each individual.

This customized care plan could include important services such as:

Medical and Therapeutic Care

Frequency of services
(weekly, bi-weekly, etc.)

Medical equipment needed
(IV, oxygen, etc.)

Treatment goals (increased mobility & return to activities of daily living)

At Home with Shepherd staff will keep your doctor updated about your progress and inform you
about any doctor-approved changes in the care plan.

Home Care Visit Expectations


Our staff provides necessary medical care and will monitor your well-being. Some things individuals can expect our staff to do during a visit include:

  • Check vital signs (blood pressure, temperature, heart rate and breathing)
  • Review medications and treatments to ensure they are being taken and used correctly
  • Monitor and address any pain
  • Ensure that the individual is in a safe environment
  • Teach the individual or their caregiver about their illness or injury and teach self-care strategies
  • Monitor the individual’s diet

At Home with Shepherd Eligibility


Home health benefits can be used for those meeting the following criteria:

The individual must be under the care of a doctor who must make a referral on their behalf for home health services.

The individual’s doctor must certify in this referral that they are in need of one or more of the following:

  • Intermittent skilled care
  • Physical therapy
  • Speech therapy
  • Occupational therapy

The individual must be homebound as certified by their doctor. This means that their medical condition(s) creates a taxing effort to leave their home and prevents them from leaving home without significant help. They may leave home for medical treatment or for short times such as for church or adult day care.

If you feel that you or a loved one may benefit from home health services from Shepherd, please consult with your doctor. With your physician’s referral, At Home with Shepherd will schedule an appointment to come to your home to answer your questions and to discuss your needs and treatment goals.

Medical Services

Skilled Nursing Care


Our experienced staff of registered and licensed practical nurses provide the highest level of medical care from taking blood pressure readings to administering IV medications. They also train individuals and family members on how to follow in-home health care procedures.

Rehabilitation


To help individuals regain maximum independence, we work together with their physician to create a personalized, in-home program for physical therapy, occupational therapy and/or speech therapy. The frequency and duration of these services are customized for each individual’s care plan.

Social Services


Access to community resources can be a vital tool for an individual’s ongoing recovery and care needs. At Home with Shepherd helps connect individuals with resources such as transportation and meal services that will help them as they regain independence.

Personal Care Services

Private Duty Nursing


At Home with Shepherd offers Private Duty Nursing for those seeking services such as bloodwork, routine assessments or medication management. Working together with their physician, Shepherd’s care team provides in-home support for individuals with a range of home care needs.
